Review Article Success Rate and Patient Satisfaction with the Essure® Sterilization in an Outpatient Setting: A Prospective Study of 857 Women. Mino M, Arjona JE, Cordon J, Pelegrin B, Povedano B, Chacon E. BJOG 2007;114:763-6. Review
Eight hundred fifty-seven women underwent Essure® sterilization by two physicians in an outpatient setting between 2003 and 2005. Ibuprofen and diazepam were administered one hour pre-procedure. Paracervical blocks were utilized routinely in the first half of the study but then only selectively. The mean procedure time was 6.8 minutes with a successful insertion rate of 98.6%, though 14.9% were considered to be very difficult. Only 3.8% of patients reported more pain than a normal menses, 8.9% used oral analgesics following the procedure, and 0.7% took off more than one day from work. All patients rated their satisfaction as high or very high. A pelvic X-ray was performed 3 months post-procedure to check placement of the device and HSG was required in 8.9% that were inconclusive. Comments In this large prospective study, hysteroscopic sterilization with Essure® in an outpatient setting was found to be quick, highly successful and well tolerated. Other studies noted it to have very low contraceptive failure rates. For these reasons, it is gaining popularity. The advantages over laparoscopic sterilization must be weighed against the fact that it is not reversible.
